DESCRIPTION: This is a combined synopsis solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented by additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Proposals are being requested for Request for Proposal (RFQ), VA528-16-Q-0005 for the following:





STATEMENT OF WORK

Sidewalk salt is needed for the Buffalo and Batavia VA Medical Centers for the winter of 2015-16 with a period of performance being October 2015 to April 2016. Salt is to be delivered in 50lb bags on pallets with a total quantity not to exceed 250 bags. These will be ordered on an as needed basis throughout the winter months. Salt needs to be environmentally friendly, non-slippery on floors and made from a Calcium blend (calcium chloride, magnesium chloride, potassium chloride and sodium).



Facility Delivery hours are between 6:00 Am and 2:00 PM Monday through Friday.

VA Medical Center Batavia - 222 Richmond, Batavia, NY



SYNOPSIS/SOLICITATION 

Proposals must be received no later than 9:00 AM, EST on Friday, October 9, 2016. Offers may be submitted via email to Sam DeMuzio, sam.demuzio@va.gov. When submitting offers, please note the solicitation number VA528-16-Q-0005 in subject line of the email. Please note that the offeror assumes all risk for any delay in the transmission of their proposals. Any offers received after the 9:00 AM EST deadline will not be considered.
