This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification System Code is 339112, Surgical and Medical Instrument Manufacturing. The small business size standard is 500 Employees or less. This procurement is a total set-aside for small business concerns. 



SPECIFIC CONTRACT REQUIREMENTS: Contractor shall furnish supplies/services to the Richard L. Roudebush VA Medical Center, 1481 W. 10th Street, Indianapolis, IN, 46202. Delivery is 15 days after receipt of order. Proposal in accordance with Request For Quote (RFQ) VA-251-14-Q-0349.

The provision at 52.212-1, Instructions to Offerors - Commercial (JUL 2013), applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this procurement. Quotes shall be evaluated on price and past performance. Price is significantly more important than past performance. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (APR 2013), with its offer.
