Chemicals & chemical products NAICS Code: 325 -- Chemical Manufacturing/325998 -- All Other Miscellaneous Chemical Product and Preparation Manufacturing 0031 is issued as a Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-83 (2 JUL 2015). The Government contemplates award of a Firm-Fixed Price Contract for The United States Army, Public Health Center - Provisional (APHC (prov)) of several different types of chemicals on an, "as needed" basis. These chemicals will be used by the Laboratory Sciences Directorate to perform sample analysis on environmental, occupational health and diagnostic samples. See Attachments. (Attachment1) - Chemicals Requirement list (Attachment2) - Performance Work Statement. This solicitation is set aside as an unrestricted, Brand name only. The Associated North America Industry Classification Code (NAICS) is 325998 and the Business Size Standard is 500. This acquisition is in accordance with FAR Part 13. Contractor shall provide a unit price for each of the items listed in Attachment1 - Chemicals Requirement list. The government has a BRAND NAME ONLY requirement as specified in Attachment 1. Acceptance shall be at destination. Shipping shall be FOB Destination to US Army Public Health Command, ATTN: MCHB-TS-LOD/CSA, Bush River Road, Building E-2100, Gunpowder, MD 21010-5403. Description of the Requirement: CLIN 0001: Chemical Supplies is required in accordance to (Attachment1) - Chemicals Requirement list and (Attachment2) - Performance Work Statement. The provision at FAR 52.212-2 - Evaluation -- Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The lowest price technically acceptable (LPTA) process described in FAR 15.101-2 will be used, meaning the award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. The technical evaluation will be a determination based on information furnished by the Contractor. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. In order to be considered technically acceptable, Contractor responses must provide: a. Information in enough detail to show that the Contractor is able to meet the requirements of the Statement of Work and Requirements List; b. Point of contact name and contact information, company CAGE code, DUNS number, and TIN. Partial quotes will not be evaluated by the Government.
