Medical services NAICS Code: 561 -- Administrative and Support Services/561320 -- Temporary Help Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFP) 2016-029-067-0002. This solicitation is issued on an unrestricted basis. (iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. (iv) The associated North American Industry Classification System code is 561320 and the small business size standard is $27.5 million. (v) PRICE/COST SCHEDULE - Licensed Optometrist services. (vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide Licensed Optometrist services in support of direct patient care for the Inscription House Health Center, Tonalea, AZ. (vii) Date and Place of Delivery: To be determined based on approval of Credentialing; Inscription House Health Service, Highway 98, Navajo Route 16, Tonalea, AZ, 86044. (viii) PERFORMANCE WORK STATEMENT: The government requires Licensed Optometrist Services in support of direct patient care. The contractor shall provide comprehensive eye care to patients seen in the IHHC Eye Clinic, including the diagnosis and management of visual and ocular pathologies through external and internal physical evaluation of the eye and adnexa. See Attached PWS. (ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: past performance, qualifications of each proposed candidate, and prior IHS experience. These evaluation factors will be weighted more than price. As part of the evaluation process, candidates may be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. (x) Copies of: (1) Certificate(s); (2) License; and (3) Insurance. PROVISIONS AND CLAUSES: The following provisions and clauses apply to this acquisition. The FAR provision 52.212-1 Instructions to Offerors Commercial Items; and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference. CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. PRO-CHILDREN ACT OF 1994: The Contractor certifies that it will comply with the provisions of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
