Office supplies and devices NAICS Code: 337 -- Furniture and Related Product Manufacturing/337214 -- Office Furniture (except Wood) Manufacturing STATEMENT OF WORK Project Title: Disassembly and Reassembly of Haworth Premise Series Cubicles for the Health Resource Center (HRC) Place of Performance: The work will be performed at HRC's Topeka, Kansas location. The address is: VA Health Resource Center 3401 SW 21st Street, Building 9 Topeka, Kansas 66604 Requirements: NOTE: The cubicles are the Premise series manufactured by Haworth Industries. All work will be done during normal working hours 7:30AM to 4:00PM Monday through Friday. Estimated time frames for this work to take place are stated below in parentheses. Firm dates will be established after contract award. Part 1 and Part 2 of the SOW are expected to occur in FY16, and Part 3 is expected to take place in FY17. Part 1 1. Disassemble and remove 24 cubicles in Room A101 and place in storage containers on the VA campus. The storage containers will be provided by others. This will be before construction begins in this area. (11/15-12/15) 2. Move all existing disassembled cubicle parts that are presently in Bldg. 9 storage area to storage containers provided by others. 3. Re-assemble 24 cubicles in Room A101 from the storage containers. This work coordinated with the HRC before at the end of construction. The cubicles will have to be connected to power and data by other contractors. (3/16-4/16) Part 2 1. Disassemble and remove 27 cubicles from the ground floor B-side north and south wings, and 4 cubicles from the large east wing and place in the storage containers on campus. (5/16-6/16) Part 3 1. Disassemble and remove 25 cubicles from the ground floor B-side east wing and place in storage containers on campus. (1/17-2/17) 2. Re-assemble 47 cubicles in the east wing and 12 cubicles in the north wing of ground floor B-side from storage. Insure that there are enough parts to complete the cubicle installation before work begins. This work will have to be coordinated with the HRC before the end of construction. The cubicles will have to be connected to power and data by other contractors.
